If you have not already sent us your ST3 form, please do so now.
Step 2: Submit Your ST3 FORM:
The Form ST3, Certificate of Exemption, can be found by clicking the button below. After completing the form, please return the first page by email to [email protected] or fax it to (651) 450-0160. Once approved, we can direct you to our online wholesale store and send you additional information about our products and prices.
Step 3: Wait for Approval
Once you have been approved by our staff, you will be able to login and see our Online Wholesale Store.
Access to our store allows you to view all of our products and prices, and place your orders online. You may also request that your business be added to our Providers map to help customers find you.
Step 4: Setup Your Online Store
(Optional)
When we’ve received your form and you’ve registered for our store, you will be eligible for your own online retail store to offer Forever Pets products. We manage the creation and hosting of the site, without any upfront or ongoing costs to you. If you want to know more, you can request information from us at [email protected].
If you already have your own eCommerce site, we would be happy to provide you with product descriptions or images to begin offering Forever Pets products.
